How to Request Your Consumer Report
To request your Consumer Report with Clarity Services, Inc., please review and complete the following forms:

We will not begin processing your request until these forms are fully completed and returned to Clarity. Your signature and a photocopy of your driver's license is required; they must be included before your request can be processed.
The form will not be considered complete until you sign your name and provide a copy of your drivers license. Please note that your drivers license must be in a readable format:

Clarity Services, Inc will investigate your inquiry directly with the third party service provider or internally and respond to you within 30 to 45 days by United States mail.
In connection with your trade line information, Clarity Services, Inc. may receive information about you from a third party source; this might include information about you past credit history, banking history, or personal identification.
Once your request has been processed and you have received your Clarity Services, Inc. Consumer Report, please review it carefully. If you find any errors, please notify Clarity Services immediately by completing the Reinvestigation Form, which will be included with your Consumer Report.
